Abstract

Introduction: Hepatocellular carcinoma (HCC) has been related to Hepatitis B, C, type 2 diabetes mellitus (T2DM), obesity and male sex. Although in half of the cases it occurs in a non-cirrhotic liver. The aim of this work was to demonstrate the epidemiological characteristics associated with HCC in a single center of study.

Methods: The present retrospective observational study was carried out in male patients diagnosed histologically of HCC between 2009-2013 in the Hospital of Solca- Guayaquil. Variables, age, origin, occupation, etiology, presence of cirrhosis and type of treatment were measured. Cases were analyzed comparing patients with and without cirrhosis.

Results: 67 cases of patients with HCC with a mean of 64.6 years entered the study. 43.3 % of patients belonging to the province of Guayas, 32.8 % of workers by profession. In the cirrhosis group (n = 38) the Hepatitis B Virus with 13 cases (19.4 %) in the group without cirrhosis (n = 29) were 6 cases (9 %) P = 0.22. T2DM + Obesity in the cirrhosis group 1 case (2.6 %) in the group without cirrhosis 14 cases (20.9 %) Odds Ratio 0.029 (95% CI 0.0035 to 0.24) P <0.001. Alcoholic habit 5 cases (7.5 %) in the Cirrhosis group and 0 cases in the group without Cirrhosis RR 1.88 (95% CI 1.49 to 2.37) P = 0.04.

Conclusion: Hepatitis B is the main prevalent factor in patients with HCC, followed by the presence of Type 2 Diabetes Mellitus together with Obesity. In patients without cirrhosis the first explanation of the etiology of HCC is T2DM together with the presence of a Obesity. The alcoholic habit is a statistically significant risk factor in the group of patients who presented liver cirrhosis and HCC
